Our office is open from 8:00 a.m. until 4:30 p.m. Monday-Thursday and 8:00 a.m. until 12:30 p.m. on Friday.

Our telephone is answered by an operator who will direct your call to the appropriate department. If the party you are trying to reach is on another line, you may be asked to hold or we will be happy to return your call as soon as possible.
Our appointment department is happy to schedule all clinic appointments, handle cancellations, and appointment time inquiries.
Our surgical coordinator will schedule all surgeries and handle all pre-surgical inquiries.

If an eye care emergency occurs, please call our office 24 hours per day at 595-4144 (local) or 1-800-888-9145 (long distance). During regular office hours our staff will assist you. After office hours, the answering service will notify the doctor on call immediately.
If you have questions about your eyes or medications, our phone nurse will assist you. If you need a refill on your eye medication, please call your pharmacy and they will call our office for authorization.
Please plan to be at our office 1 1/2 to 3 hours on your initial visit. If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. The courtesy allows us to be of service to other patients.
Payment for routine office visits is expected at the time services are rendered. We accept payment by cash, check, Visa, Master Card, Discover and American Express. We do not routinely file insurance claims for office visits. However, we will provide you with an itemized statement to forward to your insurance company. We accept assignment of Medicare (Medicare's approved fee schedule). You are responsible for any charges Medicare does not pay, such as deductibles and co-payments. Additionally you are responsible for any non-covered services, such as refractions. We will gladly file insurance claims for surgical procedures.
All your medical records and any information you can give any staff member are confidential. No information about you or your medical history will be released unless we have a written authorization from you to do so.
